After you fill out this Order Request the Marketing Team will send your choices to our vendor who will fulfill the order. You’ll have to fill out a form for each person you want to receive a box.
Things to note:
- Only use physical addresses (no PO boxes)
- Fill out one form per box recipient
- Keep message short as it will be printed on a small card
